Online Psychiatry & Telepsychiatry in California: Mental Health Care That Is Easier to Continue

Getting psychiatric care does not always require traveling to a clinic for every appointment. For some patients, video-based psychiatric evaluation, treatment discussions, and follow-up visits can reduce travel and scheduling barriers while making it easier to stay engaged in care.

Insight Choices offers telepsychiatry for patients located in California, along with in-person psychiatric services. Our psychiatric care team includes psychiatrists, psychiatric nurse practitioners, physician assistants, and other mental health professionals. Care is individualized based on your symptoms, medical and psychiatric history, prior treatment, current medications, daily functioning, and treatment goals.

Telehealth is not appropriate for every person or every situation. Whether an evaluation or follow-up visit can be completed remotely depends on where you are physically located at the time of the appointment, your clinical needs and safety, the clinician’s authority to practice and prescribe, and applicable laws and regulations. When needed, your clinician may recommend an in-person visit, additional testing, or a different level of care.

What Is Online Psychiatry / Telepsychiatry?

Telepsychiatry is psychiatric care delivered through remote communication technology, most commonly a live video visit. The location of the appointment is different, but the clinical work still involves understanding symptoms, history, current functioning, treatment response, and safety concerns, then discussing a diagnostic impression and reasonable treatment options.

The Medical Board of California describes telehealth as a tool for practicing medicine rather than a separate form of medicine. Physicians treating patients through telehealth remain responsible for the same standard of care and for applicable requirements involving informed consent, privacy, and professional responsibilities.

What Psychiatric Services Can Be Provided Through Telehealth?

Depending on clinical needs and eligibility, telepsychiatry may be used for:

Initial or comprehensive psychiatric evaluation

Reviewing symptoms, duration, severity, medical and psychiatric history, prior treatment, and daily functioning.

Medication management and follow-up

Evaluating medication effectiveness, side effects, adherence, and whether the treatment plan needs adjustment.

Symptom and functional monitoring

Following changes in mood, anxiety, sleep, attention, daily functioning, and other treatment-related concerns.

Standardized rating scales and clinical monitoring

Using appropriate measures to support assessment and track change over time. Rating scales do not determine a diagnosis on their own.

Treatment planning

Discussing medication, psychotherapy, lifestyle support, or other treatment options based on the clinical picture and individual needs.

Ongoing care and coordination

Arranging follow-up care and, when appropriate, coordinating with psychotherapy or other health care services.

Not every service can or should be completed online. If you need a physical examination, vital signs, laboratory testing, another diagnostic procedure, or a more direct assessment than telehealth can safely provide, an in-person visit may be recommended.

Who May Be a Good Fit for Telepsychiatry?

Telepsychiatry may be useful for people who want to reduce travel time, have demanding work or school schedules, live farther from a clinic, or already have a treatment plan and need ongoing follow-up.

You may be able to consider telehealth if:

You are physically located in California at the time of your appointment and meet the applicable eligibility requirements.

You are seeking professional evaluation for depression, anxiety, attention problems, sleep concerns, mood changes, or other mental health symptoms.

You are already receiving psychiatric care and need monitoring of symptoms, medication response, or side effects.

You have access to a reasonably private setting where you can speak openly.

You have a phone, tablet, or computer that can support video visits and a reasonably stable internet connection.

These factors do not determine telehealth eligibility by themselves. If your clinician believes an in-person evaluation would provide safer or more complete care, other arrangements may be recommended.

When Might In-Person Care or a Higher Level of Care Be More Appropriate?

Telepsychiatry can improve access, but convenience should not replace safety. In-person evaluation, additional medical assessment, or a higher level of care may be appropriate when:

Symptoms are worsening rapidly or cannot be adequately and safely assessed through telehealth.

A physical examination, vital signs, laboratory testing, or another diagnostic procedure is needed.

A treatment or medication decision requires additional monitoring, records, testing, or face-to-face assessment.

Privacy is not adequate or technology problems significantly interfere with clinical communication.

Your clinician determines that an in-person visit is more appropriate based on your history, symptoms, or safety needs.

If you or someone around you is in immediate danger, may harm yourself or others, or needs emergency medical attention, do not wait for a routine telehealth appointment. Call 911 or go to the nearest emergency department. If you are experiencing suicidal thoughts, severe emotional distress, or need immediate crisis support in the United States, call or text 988 to reach the 988 Suicide & Crisis Lifeline.

What Happens at Your First Online Psychiatry Appointment?

Complete New Patient Registration

New patients can begin with the Insight Choices registration process. If you plan to use insurance, the current registration process asks for insurance information and California identification, along with other required information.

Complete Intake and Service Forms

Before the initial assessment, patients are generally asked to complete a Clinical Intake Form, Service Agreement, and behavioral health questionnaire. These materials help the clinical team understand your background before the visit.

Meet With a Psychiatric Clinician by Video

During the appointment, your clinician may ask about your main concerns, when symptoms began, how often they occur, how severe they are, and how they affect sleep, work, school, relationships, and daily functioning. Depending on the situation, the clinician may also review prior mental health treatment, medical conditions, prescription and over-the-counter medications, supplements, substance use, and relevant family history.

Discuss the Clinical Impression and Treatment Direction

After the evaluation, your clinician will discuss the current clinical picture, whether more information or additional evaluation is needed, and reasonable treatment options. A first appointment does not always establish every diagnosis, and it does not automatically lead to a prescription.

Plan Follow-Up Care

Mental health treatment often requires continued monitoring. Follow-up may occur through telehealth or in person depending on symptom changes, treatment response, side effects, safety, and clinical needs.

Medication Management Through Telepsychiatry

An online psychiatry appointment is not simply a way to “get medication online.” Medication should be prescribed only when a qualified prescribing clinician determines that it is clinically appropriate after an adequate assessment.
When medication is part of the treatment plan, follow-up may include:

Whether symptoms are improving and to what degree.
New or ongoing side effects.
How medication is actually being taken, including missed doses or other adherence concerns.
Other prescription medications, over-the-counter medications, supplements, alcohol, or substance use that may affect treatment.
Whether the dose, medication, treatment strategy, monitoring, or additional evaluation should change.

Whether a particular medication can be prescribed or continued through telehealth depends on the diagnosis, the patient’s location, the clinician’s prescribing authority, safety considerations, and applicable state and federal requirements. Some situations may require additional information or an in-person assessment. Do not stop prescribed medication or change the dose without guidance from a qualified health care professional.

How Is Privacy Protected During Telehealth?

Mental health visits often involve highly sensitive personal information. Telehealth, like in-person care, is subject to applicable health information privacy and security requirements. Patients can also take practical steps to reduce privacy risks when attending appointments from home or another location.

Before your appointment, consider:

Choose a private room or space where other people are unlikely to overhear.

If another person is present, decide whether you are comfortable having them remain during the visit.

Use headphones when possible.

Avoid public Wi-Fi or shared devices when accessing health information.

Test your camera, microphone, internet connection, and device battery beforehand.

Prepare a current medication list, relevant medical information, and the questions you want to discuss.
